Failures in its execution are associated with increased morbidity and mortality and administrative consequences, such as surgery cancellations, which reinforces the need to optimize this practice. Objectives: To analyze the contribution of PAA to patient safety in elective surgeries, identifying its indispensable components and evaluating how the observed risks and comorbidities impact anesthetic planning. Methodology: An integrative literature review was conducted in the SciELO, PubMed, and Scopus databases, with articles published between 2020 and 2024. The search used the descriptors "assessment," "pre-anesthetic," "surgery," and "elective," and the selection of studies followed the stages of title, abstract, and full-text analysis. Results: The search resulted in a final sample of 14 articles (2 from SciELO, 5 from PubMed, and 7 from Scopus), the contents of which were used for the analysis. Discussion: A quality PAA requires a targeted medical history and physical examination, with airway assessment (Mallampati score, STOP-BANG) and a personalized approach for at-risk populations. The assessment transforms anesthetic planning from reactive to proactive, as the identification of specific risks (difficult airway, OSA, hypotension) allows for the implementation of preventive and customized strategies, including the use of technologies and the appropriate selection of drugs. Conclusion: The PAA is consolidated as an essential investigative process for patient safety.
