Acesso em:.https://bdm.ufpa.br/handle/prefix/9386Upper gastrointestinal bleeding (UGIB) represents a medical emergency with high morbidity and mortality, especially in remote regions such as the Xingu Region of Pará. This study aimed to describe the challenges faced in the diagnosis and access to endoscopic therapy for UGIB in this region, through a narrative literature review using the PubMed and SciELO databases. Nine studies published between 2019 and 2025 were selected, addressing clinical aspects, structural barriers, and the impacts of delayed treatment. The results indicate that, despite the established protocol for the management of UGIB, geographic factors, equipment shortages, and a lack of specialized professionals hinder timely access to upper gastrointestinal endoscopy in the Xingu Region. This limitation contributes to increased morbidity, need for transfusions, and mortality. Local training, implementation of mobile units, and improved infrastructure are proposed to minimize these challenges. Furthermore, primary prevention through controlling risk factors, such as the inappropriate use of nonsteroidal anti-inflammatory drugs (NSAIDs), is essential.
