This study aimed to evaluate the regulatory and operational landscape of Reverse Logistics for Expired or Unused Household Pharmaceuticals at the federal, state, and municipal levels, focusing on suggesting regulations for the municipality of Tucuru´ı. The research methodology consisted of three stages: the first was mapping the current regulations at the federal, state, and municipal levels regarding the Reverse Logistics of Medicines; the second consisted of a comparative analysis by technical axes of the regulations found; and the third was the proposal of mitigating measures. The results showed that despite the progress of the National Solid Waste Policy (PNRS) and State Law No. 9,898/2023, Tucuruí still lacks local regulations, which ultimately weakens the success of Reverse Logistics of Medicines in the municipality, exposing water resources and the local ecosystem to environmental liabilities such as bacterial resistance and endocrine disruption of fauna. The research presented proposals for mitigating measures, such as the creation of municipal regulations that could use the regulations of municipalities like Belém, Santarém, and Capanema as a reference; the creation of intermunicipal consortia to reduce logistical transportation costs; and the creation of environmental education campaigns.
