In the Xingu region of Pará, Brazil, the scarcity of healthcare services and socioeconomic inequalities exacerbate the situation. This study aimed to evaluate the QoL of hemodialysis patients at the Hospital Regional Público da Transamazônica, the only reference center in the region. Objectives: To characterize the sociodemographic and clinical profile of patients, identify the most compromised QoL domains, and analyze the relationship between socioeconomic, clinical variables, and QoL. Methods: A cross-sectional study was conducted with 85 hemodialysis patients using the KDQOL-SF™ instrument and a sociodemographic questionnaire. Non-parametric tests (Mann-Whitney, Kruskal-Wallis) and Spearman correlation (p<0.05) were applied. Results: Most patients were male (61.18%), aged 40-59 years (56.47%), and had a family income ≤2 minimum wages (65.88%). The most compromised domains were work status (10.59), cognitive function (16.47), and social interaction (25.33). Significant correlations were observed between low income and worse QoL in symptoms (p=0.0092) and disease effects (p=0.0022), as well as an association between higher education and better perception of disease effects (p=0.0404). Conclusion: The QoL of hemodialysis patients in Xingu is deeply affected by social determinants, such as insufficient income and limited access to education. The implementation of public policies and multidisciplinary interventions is recommended to improve comprehensive care for this population.
