The aim was to analyze the evolution of bone mineral density in PLWH. Thus, medical record data and bone densitometry (DXA) results were collected from an initial and final moment; patients over 18 years old, using antiretroviral therapy (ART) treated between 2017 and 2024, at the outpatient clinic of the Núcleo de Medicina Tropical (NMT), in a city in northern Brazil, were included. Data from 53 PLWHA were analyzed. Mean age was 59.4 years, male prevalence (52.8%), median CD4 T lymphocyte count was 754 cells/cm², predominance of undetectable viral load (92.5 %), high prevalence of tenofovir use (66%) and HIV infection and ART average time were 18.83 and 18.37 years, respectively. Main comorbidities were dyslipidemia (79,2%) and lipodystrophy (58,5%). In the sample, there was a prevalence of low bone mineral density (osteopenia and osteoporosis), with 67,9% in the initial and 62.2% in the final evaluation, and hypovitaminosis D (60.4%). Risk factors associated with low bone mineral density in PLWH were older age (p= 0.002) and female gender (p= 0.019). Comparing the initial and final DXA of each patient, 75.5% showed improvement or stability in bone mineral density. Lumbar spine T-score values improved (p= 0.007), with a percentage increase of 15.6%.
