In this context, regular physical activity acts as a protective factor, providing physical, psychological, and social benefits, as well as reducing symptoms of stress, anxiety, and depression. Therefore, understanding this relationship is essential to strengthen health promotion and well-being strategies during medical education. Objectives: To identify the effects of physical activity practice on the quality of life of medical students. Methods: A systematic review was conducted with searches performed in the Web of Science, Scopus, PubMed, and LILACS databases, using health-related descriptors for medical students, physical activity, and quality of life. Study selection, data extraction, and risk of bias assessment were independently carried out in parallel by two authors, with conflicts resolved by consensus. Subsequently, a narrative synthesis of the results was performed. Results: Seven cross-sectional studies were included, conducted in Brazil, the United States, Pakistan, Indonesia, and Iran, with a mean of 1,162.7 participants per study, predominantly female (59.7%). Physical activity practice was positively associated with quality of life and lower stress levels among medical students. The studies presented important methodological differences that prevent more appropriate comparisons. Methodological quality was generally rated as high risk of bias, mainly due to issues related to participant recruitment methods for sample composition and unclear measures of physical activity. Conclusions: Physical activity is positively associated with the quality of life of medical students, with evidence of a dose-dependent relationship between exercise intensity/duration and better outcomes. Female students show lower adherence to physical activity and worse quality of life perception. In this context, universities should implement programs that encourage regular exercise as a strategy for mental health care and well-being.
