In recent years, several herbal medicines have been studied for the prevention and treatment of oral mucositis due to their antioxidant and anti-inflammatory properties. In this regard, a systematic review was conducted to analyze the effectiveness of herbal medicine in the prevention and treatment of oral mucositis in patients undergoing cancer treatment. The research was conducted using the PubMed, Embase, Scopus, ScienceDirect, Web of Science, and Cochrane databases. The search used MeSH terms such as Therapy Associated Neoplasms, Radiotherapy, Chemotherapy, Phytotherapy, Medicinal plants, Plant Extracts, Traditional Medicine Practitioners, Ethnopharmacology, and Oral Mucositis or Stomatitis, combined with the Boolean operators AND and OR. Among the 792 studies identified in the electronic databases, only 27 randomized clinical trials were included, reporting a total of 1,311 participants who used herbal medicines for the prevention and treatment of oral mucositis. The risk of bias assessment was conducted using the Cochrane Handbook (Review Manager). Thirteen studies were evaluated as having a low overall risk of bias (48.2%), twelve (44.4%) presented a moderate risk of bias, and two clinical trials (7.4%) showed a high overall risk of bias. According to the present study, it was possible to identify that plants such as Chamomilla Recutita, Aloe vera and Curcuma longa L. show potential as a preventive and complementary treatment for oral mucositis due to their anti-inflammatory effects. Despite the limited number of clinical trials in humans, these substances did not demonstrate significant adverse effects in patients. In conclusion, we found that the potential of herbal medicines as adjuvants is valuable in the treatment of oral mucositis, opening new horizons for future research and clinical practice in the relentless search for more effective, safer therapies with better acceptance by cancer patients.
