Almost 14 millions of diabetics in the United States, there are a third suffers with Painful Diabetic Polyneuropathy. There is no consensus or therapeutic algorithm regarding the initial analgesic therapy or, clinical studies evidencing real superiority of one of the therapeutic modalities. The ideal treatment would be that able to relief pain with the tolerable minor dose. In this research, we present a case of Painful Diabetic Polyneuropathy that was unresponsive to usual therapies, treated with Topiramate, that presented significant improves with complete remission of painful condition after treatment with this medication. Topiramate is an anticonvulsant that present several mechanisms still little well-known, important to the painful condition treatment.
