Acesso em:.https://bdm.ufpa.br/handle/prefix/4790The case report describes a patient with Hereditary Spherocytosis (HS) that was benefited by the use of recombinant human erythropoietin, reducing the transfusional needs and it risks, presenting good erythropoietic response, till splenectomy formal indication. Hereditary Spherocytosis is caucasian most common congenital hemolytic anemia. It morphological mark is the microspherocyt, a result of quantitative and/or qualitative alterations in erythrocyte membrane proteins. In most affected families, the HS is transmitted as an autosomal dominant disorder, as mutation or a penetrance variableness of the transmitter gene, however, it can be clinically silent for indefinite periods or it can appears at three clinical forms: light, moderate and sever. It is characterized by an anemia of variable gravity, with intermittent jaundice, spherocytes presence in peripheral blood smear, increased osmotic fragility and splenomegaly. The most objective attitude as therapeutic conduct, is the splenectomy indication, however, it is not recommendable for 5 years old children or younger ones, in order to avoid infectious process of bad prognostic. Until erythropoietic response grows enough to compensate the hemolysis levels, red cell concentrate transfusions are necessary, although, a sustained response is not obtained, considering it a therapy of transitory response.
