A total of 371 adult radiographs were analyzed, classified according to image quality, and examined for the identification of discrete bone loss. The overall prevalence of early alterations was 50.4% (95% CI: 45.3–55.5%). A significant association was observed between the presence of alterations and the male sex (p < 0.001), as well as with age group, with a higher frequency in young adults (p < 0.001). Image quality markedly influenced the detection of findings, with good quality radiographs showing a higher probability of identifying early alterations (OR = 16.2; 95% CI: 7.18–36.4). Intra-examiner agreement was substantial (Kappa = 0.760; 95% CI: 0.70–0.82), demonstrating consistency in the evaluation. The results indicate that panoramic radiography, despite its limitations, constitutes a useful and accessible tool for screening early periodontal alterations, especially in contexts of high demand and limited resources.Acesso AbertoDiagnóstico precoceEpidemiologiaPeriodontitePrevalênciaRadiografia panorâmicaSaúde bucalEarly diagnosisEpidemiologyPeriodontitisPrevalencePanoramic radiographyOral healthCNPQ::CIENCIAS DA SAUDE::ODONTOLOGIA::RADIOLOGIA ODONTOLOGICAPrevalência de alterações periodontais iniciais em radiografias panorâmicas de rotinaTrabalho de Curso - Graduação - MonografiaAttribution-NonCommercial-NoDerivs 3.0 Brazil
