The acquisition of medical and hospital equipment (MHE) is a complex and strategic process for hospitals, impacting the quality of care, patient safety, and financial sustainability. This work aims to describe and analyze the MHE acquisition process, focusing on the central role of clinical engineering as the technical foundation for this activity. The research, based on an extensive literature review, demonstrates that acquisitions are often based solely on the lowest equipment price, resulting in significant losses for hospitals, which can reach the order of millions of reais over the equipment's life cycle, due both to high maintenance and replacement costs and to the direct risks to patient safety and well-being. The study details the process from the identification of the clinical need to the formalization of the purchase, emphasizing technical criteria such as risk analysis, technological suitability, and total cost of ownership.
