Epithelial-mesenchymaltransition (EMT) is a biochemical process in which epithelial cells acquire a mesenchymalphenotype, leading to enhanced migration, invasiveness, apoptosis resistance, andmetastasis.MicroRNAs (miRNAs) are small, endogenous, non-coding RNAs that can act as oncogenesor tumor suppressors when deregulated. Objective: This systematic reviewaimedtoevaluatethe relationship between microRNA deregulation and the occurrence of the metastaticprocessassociated with EMT in OSCC. Materials and Methods: The Preferred ReportingItemsforSystematic Reviews and Meta-Analyses (PRISMA) guidelines were followed. Searcheswereconducted in the PubMed, Web of Science, LILACS, Scopus, Embase, Google Scholar, andEBSCO Teses databases. No restrictions on year or language were applied. Thereviewprotocol was developed and registered in the PROSPERO database (University of York, York,United Kingdom) under registration number CRD42024531676. Results: Eighteenstudieswere included in this review. A total of 17 miRNAs involved in the regulatory mechanismofEMT were identified, of which five were upregulated and twelve downregulatedinOSCCtissues. Furthermore, eight deregulated miRNAs showed a significant association withlymphnode metastasis, and four showed a significant correlation with distant metastasis.Conclusion: Despite the complex regulatory mechanisms of these molecules, thestudiesincluded in this review highlight the potential of miRNAs as targets for diagnosis andtherapyin the clinical management of OSCC.
