Acesso em:.https://bdm.ufpa.br/handle/prefix/10055Success in endodontic therapy requires a basic understanding of the morphological anatomy of the root canal system, particularly of the permanent upper first molar. It presents, in its most common form, three or four root canals, with the commonly found location of the fourth channel being at the mesiobuccal root, slightly behind the mesio buccalcanal. The objective of this work is to report the treatment of a permanent upper molar with an anatomical variation showing 4 root canals, being mesiovestibular, mesiopalatal, distopalatine and distovestibular. The morphological variation in the number of root canals is not uncommon in the clinical routine, however it deserves attention, since the access, modeling and filling of all canals are fundamental for success in endodontic treatment.
