This narrative review aimed to compile up-to-date scientific evidence on the main clinical methods for pulpal diagnosis, with emphasis on the accuracy of thermal, electrical, and circulatory tests. Publications indexed in SciELO, PubMed and LILACS, between 2015 and 2025 were analyzed using descriptors such as cold test, electric pulp test, heat test, laser Doppler flowmetry, and pulse oximetry. Findings indicate that the cold test is the most balanced sensory method, showing high diagnostic accuracy and strong correlation with histological findings. The electric test proved effective in confirming neural vitality, though limited in cases of partial necrosis. The heat test demonstrated lower diagnostic precision. Physiological tests such as laser Doppler flowmetry and pulse oximetry showed superior performance, especially in complex cases, by directly assessing pulpal vascular perfusion. However, both methods present limitations: laser Doppler flowmetry is sensitive to patient movement and optical contamination and has limited clinical availability; pulse oximetry requires technical adaptation to dental structures and may be affected by restorations and ambient light.
