Considering the high prevalence of leprosy in Brazil and the state of Pará, it is essential to recognize the epidemiological aspects of the disease in the region. Objective: To analyze the epidemiological profile of leprosy cases in the state of Pará during the period from 2018 to 2022; to identify factors related to the incidence of leprosy in the microregions of the state of Pará; to describe the epidemiological characteristics of leprosy in the state of Pará; and to evaluate the temporal trend of leprosy in the state of Pará between the years 2018 and 2022. Methodology: Analysis of data available in official databases, such as the Notification Disease Information System (SINAN/TABNET) recorded during the period from January 2018 to December 2022. Results were analyzed through the calculation of incidence rates, means, Pearson's linear temporal correlation coefficient test, chi-square association test, and calculation of the 95% confidence interval (CI95%). Results: Decrease in the temporal trend of the incidence rate with a strong correlation (r = -0.85). Greater concentration of cases in the southern microregions of the state, as well as among men, mixed-race individuals, those in the economically active age group, and those with low education. The multibacillary operational class predominates. Significant reduction of new cases in individuals under 15 years of age during the studied period, with a decreasing trend showing a strong temporal correlation (r = -0.98). Conclusion: Data analysis showed that the reduction in the incidence rate aligns with the decrease in the national average, particularly since 2019, which was influenced, among other reasons, by the impact of the COVID-19 pandemic.
